Supervisor's Accident Investigation Report

If you are injured while working for Utah State Univeristy, the first priority is that you get treated. You should inform your surpervisor that you have been injured and seek treatment. If your injury is not life threatening, you should visit a clinic that participates with the Workers Compensation Fund. A list of participating providers can be found at https://www.wcf.com/preferred-medical-providers. For employees on the USU Logan Campus the provider is the Intermountain WorkMed located at 412 N. 200 E. Logan.

If your injury is life threatening, please immediately go to the nearest emergency room and seek the appropriate treatment.

You, with the help of your supervisor, will need to complete an Employer's First Report of Injury as soon as is practicable after your injury. A link to a downloadable copy of the form can be found here. Please fill out this form as best as you are able and return it along with any paperwork you have received from your physician to USU Risk Management. These documents can be emailed to risk@usu.edu or faxed to 435-797-1925. Supervisors are also requested to complete a Supervisors Incident Investigation Form that will help indentify the the causes of the injury and possible methods to prevent the injury in the future.
